Complex Form

This project was to automate a state crash report form that officers would fill out by hand. The client wanted to convert this to an online version that civilians could use in certain situations to self-report a minor car collision.

Many government agencies are still years behind in their methods of capturing information and filling out forms. In this case, we had to establish the set of rules behind the state's long-form collision report before the report could be broken down into bite-size pieces so that a civilian could provide accurate and complete information online.

Second step - Form concept

This project was a collaboration between the usual internal team (product manager, business analyst, and lead developer), a state police representative, and back-office staff who became constant invaluable contacts. We met weekly to review screens as they were fleshed out. Then took feedback for updates that we'd review the following week. There were also periods where we sought out usability feedback in the form of family, friends, and neighbors of all ages to vet and adjust the design.
